MLPC Evaluation Working Group:

Survey of Data Collection Instruments

 

In April 2007, Medical Legal Partnership (MLP) sites from around the country responded to a site survey in order to help sites get to know one another better.  One of the topics addressed in this survey was current efforts in program evaluation.  Respondents to the survey were asked to indicate whether or not their site was currently collecting data on a number of different variables related to the evaluation of direct services and health care provider education/training. The survey provided a picture of WHAT data is being collected by sites nationally.

Having captured the "WHAT" of evaluation data collection, we are now interested in the "HOW."  We are requesting that MLP sites share with us the instruments that are being used in data collection.  This may include forms completed by program staff, as well as surveys or questionnaires completed by program clients and health care providers.  Our goal is to compile instruments and indicators in a way that allows individual sites to learn from others' efforts.

Please submit each instrument or form used to evaluate your program.  Electronic copies (.doc, .pdf, etc.) are preferred and can be sent to:

Where data is entered via an electronic records system, you could print a screenshot, then fax.  For each form or instrument submitted, please answer the questions contained in this survey.  If you submit three different instruments, please complete the survey three times, with your responses each time based on one of the three instruments.
